Adult Social Care Support We Can Offer
Equity Family Service is able to provide support to Families accessing Adult Social Care under the Care Act 2014. Our services include providing support to clients who have family members in residential care homes.
We are able to liaise with care home staff and provide (daily, weekly or monthly) written and telephone feedback to clients on the emotional, mental and physical well-being of their family member residing in residential care homes.
A significant number of patients with Alzheimer's or dementia will wander from their home and may require assistance. Our dementia support service includes real-time GPS safeguarding monitoring of clients and positioning- accuracy within 5 meters. We are able to facilitate a Geo Fence with real time alerts if the service user leaves the safety of the residential home
What We Can Do

Liaising with residential care home staff to check on the welfare on resident service users
(please note, consent of the resident and the family nominating family member would have
to be obtained) -
Providing safeguarding assistive technology devices and systems that help people maintain
or improve their independence, safety, and wellbeing
What our staff cannot do:
